Please complete this form if your child is a member of Aylsham High School's Friendly Faces team. The Friendly Faces team support at various school discos across the year and we would like your consent for your child to assist at these events.

If my child is selected to participate in the Friendly face roadshow evening I understand that my child will need to be at school by 4.45pm on the evenings of the Friendly Face Roadshow.

If my child is selected to participate in the discos for friendly faces, I understand that my child will need to be at school by 6.30pm for discos.

I understand that my child will be ready to be collected from school at 8.30pm following the discos.

I will ensure that I have made arrangements for my child to get home safely following the discos.
